What This Template Covers

5 Parts

Cover page, findings, narratives, line items and a submission checklist

Key Takeaways

A roofing supplement template you can copy: cover page, findings format, narrative wording, line items and a pre-submission checklist adjusters respond to.

Preparing a supplement? Use the free checklist so nothing is missing when you submit.

Why a template saves you time and gets better results

Every supplement answers the same questions, so there is no reason to start from a blank page. A consistent template gets you three things.

Speed: Your office fills in blanks instead of composing from scratch. Consistency: The adjuster learns where to find everything, so reviews go faster. Fewer mistakes: A fixed structure makes missing items obvious.

The format below works for most residential roofing supplements. Adjust it to your carrier's requests and your state's rules. For the thinking behind it, read how to write a roofing supplement.

No email or sign-up needed. General guidance, not legal advice. Adjust it to your carrier's, client's and state's requirements.

The five parts of a supplement package

A complete package has five parts. Keep each one short and scannable.

1

Cover page: Everything an adjuster needs to file it correctly, in one place.

2

Findings summary: A short numbered list of each item and where it is.

3

Narratives and photos: One finding per block, with the narrative beside its photos.

4

Line items: A simple table of item, quantity, unit and price.

5

Certification and contact: A short sign-off and a direct way to reach you.

Send it as a single PDF when you can, so nothing gets separated.

1

Cover page

Project, address, claim number, carrier, adjuster, date of loss and inspection date.

2

Findings summary

A numbered list of every item with its location and photo numbers.

3

Narratives and photos

One short narrative and one or more labeled photos for each finding.

4

Line items

Item, quantity, unit and price, in the format the adjuster works in.

5

Certification and contact

A short statement and who to call with questions.

Capture this on the roof, not from memory.

Supplement Snap lets your crew take photos and voice notes at tear-off and drafts a report you review before sending. Your first project is free.

Cover page: what to include

Put these on the first page so the adjuster can match the document to the claim immediately:

Project name and property address

Policy holder name

Claim number and carrier

Adjuster name, email and phone

Date of loss, date of tear-off and date documented

Your company name and a contact person

A missing claim number is one of the most common reasons a package gets misfiled or delayed.

Findings format you can copy

Use the same block for every finding. Replace the bracketed text:

Finding: [Component], [location, for example front slope eave]

Condition: [What you observed, in one or two plain sentences]

When found: [During tear-off, after shingles and underlayment were removed]

Why it matters: [Water intrusion, code requirement or manufacturer requirement]

Needed: [Repair and quantity]

Photos: [Photo numbers]

Example: Finding: Decking, front slope eave. Condition: Three sheets of OSB soft and deteriorated from moisture. When found: During tear-off. Why it matters: Cannot hold fasteners. Needed: Replace three sheets. Photos: 4, 5, 6.

Keep wording neutral and factual. Avoid guessing at causes you cannot prove.

Line items and quantities

List each item with its quantity and unit, in the format your adjuster uses. A simple layout is enough:

Item description

Quantity and unit

Unit price from the price list on the claim

Total

If you build estimates in Xactimate, mirror its item names. If you do not, a clear table with measurements still works. Our Xactimate codes reference lists common items, and the units to use.

Show how you got each quantity, even if it is only a line such as 12 feet by 8 feet equals 96 square feet.

Before You Send the Supplement

Claim number, policy holder and date of loss are on every page
Every finding has at least one labeled photo
Quantities match your measurements or measurement report
Each narrative says what, where, when, why and what is needed
Code or manufacturer references are attached where you rely on them
The PDF opens cleanly and photos are legible
You know who received it and when you will follow up

Sending it and following up

Send the package by email with a clear subject line that includes the claim number and the words supplement request. In the body, list the number of findings, the total requested and what you attached.

Then follow up in writing. A short note a few days later asking whether the adjuster needs anything else keeps the file moving. If you need more guidance on disputes, see what to do when a supplement is denied.

This template is a starting point. It is general guidance, not legal advice, and your carrier may require its own forms.

Frequently asked questions

What should a roofing supplement include?

A cover page with claim details, a findings summary, a narrative and photos for each finding, line items with quantities and units, and a short certification and contact section.

Is there a standard roofing supplement format?

There is no single legal standard. Carriers differ, so use a clear consistent format and adjust to what your adjuster asks for.

How long should a supplement narrative be?

A few sentences per finding is usually enough. Say what was found, where, when, why it matters and what is needed.

Should I send photos inside the PDF or separately?

Embedding labeled photos in a single PDF keeps everything together. Some carriers also ask for original files, so keep them available.
